ACT PRIVATE HEALTH INSURANCE COVERAGE SURVEY AS AT 31 DECEMBER 2007

PHIAC requests information about the private health insurance coverage in ACT as at 31 December 2007. The results of this survey will enable health benefits funds to access information on the distribution of insured persons in ACT.

From 1 April 2007 the PHIAC 1 form provides coverage information in every State and Territory with the exception of ACT which is included in NSW.

The information that is required is found on the electronic template (survey0712.xls) in forms and attached to this circular.

Please note that the basis of the survey is the principal place of residence of the policy holder.

The completed returns should be forwarded by e-mail to
